About this rental application

Collect necessary applicant facts and document a fair, disclosed screening process before a rental decision. It is an application, not a promise that a tenancy has been approved.

When to use it

  • Use when evaluating an applicant for an identified property under published lawful screening criteria.
  • Use a separate lease only after the parties agree to the tenancy; use a separate adverse-action notice when a consumer report triggers a negative decision.

What to gather before completing it

  • Identify the property, availability, screening criteria, report provider, charge basis, and refund terms before taking an application.
  • Collect accurate rental and employment references, income sources, desired occupancy, and specific report consent.
  • Check state and local limits on fees and screening; do not collect SSNs, full bank details, or medical diagnoses in this browser form.

Common mistakes to avoid

  • Charging a default application fee in a jurisdiction that restricts or prohibits it.
  • Treating general consent as permission for any report, or using protected characteristics as selection criteria.
  • Rejecting an applicant because of a report without the required adverse-action information and dispute pathway.

After you complete the document

  • Give the applicant a receipt or copy of their application and disclosures through an appropriate private process.
  • Record the decision and relevant criteria without retaining unnecessary sensitive information.
  • If applicable, provide the separate adverse-action notice and explain the reporting company’s contact and correction process.

What the template includes

The online builder and downloaded documents use the same fields. A selected state can add relevant research prompts. Complete only applicable items; add explanations for exceptions rather than assuming a blank entry resolves them.

Practical tips

  • Be specific. A clear description is easier to verify than a broad label.
  • Download Word if you need to add an attachment or adjust the layout.
  • Keep the completed document and supporting records together.

Before signing or submitting

Check state and local rental requirements. Collect only necessary applicant information through an appropriate screening process; this form does not request a Social Security number or full bank details.

RESEARCH & REQUIRED NEXT STEPS

Federal requirements to review

Pre-1978 housing: complete the disclosure package before signing

For covered housing built before 1978, give the EPA lead pamphlet, disclose known lead hazards and available reports, and attach the required warning and signed acknowledgments. A generic lease checkbox does not supply these materials. EPA lists conditional exceptions, including certain short stays without renewal rights; record the basis for any exception. Keep signed disclosures for three years.

Assistance animals require a separate accommodation review

Do not automatically apply pet deposits, pet fees, breed restrictions, or a no-pets rule to an assistance-animal request. HUD describes reasonable accommodations and when reliable disability-related information may be requested. Use a separate accommodation record, avoid collecting a diagnosis in a public template, and evaluate the specific request rather than assuming every animal is a pet.

Tenant screening and adverse action are separate documents

If a consumer report leads to rejection, higher rent, a larger deposit, or a cosigner requirement, provide the required adverse-action notice. Identify the reporting company and explain the applicant’s rights to dispute errors and request a free report within 60 days. Application consent alone does not replace that notice. Check additional state and local screening restrictions.

Fair housing review applies to advertising and lease rules

Review screening criteria, occupancy provisions, advertisements, and house rules under federal fair housing protections and any broader state or local protections. Do not use protected characteristics as selection criteria. Identify reasonable accommodation and modification procedures separately from ordinary maintenance and rule enforcement.
