About this vehicle release form
Give a named collector limited permission to collect an identified vehicle from a named facility. This is a custody or pickup authorization: it does not sell the vehicle, transfer title, authorize an unrelated act, or substitute for government impound-release requirements.
When to use it
- Use when the lawful owner or authorized representative wants another person to collect the vehicle from a repair shop, tow/storage facility or similar custodian.
- Confirm in advance that the facility accepts owner-written authorization and ask for its own mandatory forms and identification requirements.
- For an impound, police hold, lien, estate or business-owned vehicle, obtain the applicable release approval and authority evidence before setting a pickup date.
What to gather before completing it
- Match owner and representative authority with the ownership records. Confirm the VIN, plate, claim/job number and facility address against the custodian’s record.
- Name the exact collector, their contact details, permitted pickup date and expiry. Record ID type and limited identifying digits only; provide any required full ID directly to the facility.
- Define authority narrowly: pickup, approved fee payment, receipt of keys or specified belongings, and transport. Clarify whether the collector can sign any condition or receipt acknowledgment, and identify limits.
Common mistakes to avoid
- Calling an owner pickup authorization a DMV release of liability or using it as an ownership-transfer document.
- Leaving collector identity, facility, vehicle or expiry unspecified, or assuming it defeats an official hold or unpaid lawful release condition.
- Adding a broad waiver of damage claims or disposal authority when the purpose is only collection.
After you complete the document
- Give the accepted authorization to the collector and facility, together with authority and release evidence through the facility’s required channel.
- At pickup, record vehicle condition, keys and belongings, charges paid, transport method, collector receipt and facility handover time. Note damage or missing property accurately.
- Retain the signed authorization and receipt. If the authorized pickup changes or expires, give the facility the revised authorization or revocation before collection.
