MAKE THE DOCUMENT WORK FOR YOUR SITUATION

About this pet addendum

Document ordinary pet permission and responsibilities while preserving a separate accommodation process for assistance animals.

When to use it

  • Use to add identified ordinary pets to an existing lease with the necessary parties’ consent.
  • For an assistance-animal request, record accommodation arrangements separately rather than automatically applying ordinary pet restrictions or fees.

What to gather before completing it

  • Identify the original lease, property, animals, applicable local licensing requirements, and agreed care responsibilities.
  • Review each proposed charge against state deposit rules and any total-security cap.
  • Define waste cleanup, reasonable common-area rules, reporting of damage, and a fact-based settlement process.

Common mistakes to avoid

  • Calling an assistance animal a pet and charging a pet deposit by default.
  • Treating a pet charge as automatically outside security-deposit limits.
  • Requesting a diagnosis in the form or using an unsupported blanket restriction in place of accommodation review.

After you complete the document

  • Keep the addendum with the original lease and retain copies for both parties.
  • Maintain licensing or vaccination records where appropriately required, without mixing them with private disability documentation.
  • Document condition and actual damage and follow the applicable deposit or claim process.

What the template includes

The online builder and downloaded documents use the same fields. A selected state can add relevant research prompts. Complete only applicable items; add explanations for exceptions rather than assuming a blank entry resolves them.

Practical tips

  • Be specific. A clear description is easier to verify than a broad label.
  • Download Word if you need to add an attachment or adjust the layout.
  • Keep the completed document and supporting records together.

Before signing or submitting

Check state and local rental requirements. Collect only necessary applicant information through an appropriate screening process; this form does not request a Social Security number or full bank details.

RESEARCH & REQUIRED NEXT STEPS

Federal requirements to review

Pre-1978 housing: complete the disclosure package before signing

For covered housing built before 1978, give the EPA lead pamphlet, disclose known lead hazards and available reports, and attach the required warning and signed acknowledgments. A generic lease checkbox does not supply these materials. EPA lists conditional exceptions, including certain short stays without renewal rights; record the basis for any exception. Keep signed disclosures for three years.

Assistance animals require a separate accommodation review

Do not automatically apply pet deposits, pet fees, breed restrictions, or a no-pets rule to an assistance-animal request. HUD describes reasonable accommodations and when reliable disability-related information may be requested. Use a separate accommodation record, avoid collecting a diagnosis in a public template, and evaluate the specific request rather than assuming every animal is a pet.

Fair housing review applies to advertising and lease rules

Review screening criteria, occupancy provisions, advertisements, and house rules under federal fair housing protections and any broader state or local protections. Do not use protected characteristics as selection criteria. Identify reasonable accommodation and modification procedures separately from ordinary maintenance and rule enforcement.
